I had an internal exam, ultrasound, on thursday and the twins showed strong heartbeats and looked good. On Thursday evening, I had terrible cramps in my lower abdomen and lower back and then on Friday spotting occurred. Everytime I went to the restroom, blood was present when I wiped and sometimes with small fibrous looking tissue. Today it has decreased but the cramps remain. Could this be because of the ultrasound, the fact that my period would be around this time normally, or could this be the beginnings of a miscarriage?

It is highly unlikely thaty the ultrasound caused your bleeding and cramping. The fact that your menses would have started, if you were not pregnant is unrelated. I cannot tell you why you experienced wht you did, but it is not uncommon, and the pregnancy is likely to be successful.
