What first line treatment do you suggest for your patients with PCOS?

The vast majority of my patients with PCO are insulin resistant. I have them restrict their carbohydrate intake to 30 grams a day, and take Glucophage. To actually stimulate the ovaries I use an FSH product, or Femara.
