Dr Jacobs, I am 40 years old, had my first IVF cycle in October and I am planning to start lupron on day 21 of this cycle for 2nd IVF. I just went for baseline us (no cysts) and blood test and for the first time had a high estradiol (132). Should I be concerned and view it as a sign of low ovarian reserve when I have never had had high estradiol on day 3 and my fsh has never been higher than 7? Or is this an after-effect of the first IVF? The first IVF cycle failed to get to transfer as I stimulated too quickly and only 1 egg was mature (estradiol was 1600) and did not make it to transfer. Should I still go ahead with lupron and IVF for my next cycle? Thanks for your help

An elevated estradiol on day 3 is associated with poor ovarian reserve. Please also be aware that at the age of 40, you have a greater risk of the eggs you produce having the wrong number of dhromosomes. You may wish to consider using an egg donor. Good luck.
