Hi Dr. Smith,

I had a trial transfer to get ready for my July ICSI cycle.

During the trial transfer, my RE had trouble inserting the catheter and had to try 4 different catheters before he found one that would work. I've experienced similar problems when I've had IUI's in the past.

If we encounter a problem such as this on the day of embryo transfer, could it affect the embryos and implantation?

A traumatic/difficult embryo transfer can dimish the chances of success. However, that was the point of the trial transfer - to find the correct catheter for transfer and thereby minimize problems on the day of transfer. You doctor is being concientious and thorough. I would be much more concerned if no trial transfer was performed and your doctor had to go through 4 catheters at the time of embryo transfer.

Hopefully, as you said, there won't be a problem because my RE took the time to do a trial transfer.

Is there anything else that I can do to minimize any problems that may occur during transfer? Would relaxation techniques or a tranquilizer help?

Many programs give the patient Valium or something similar, so the patient is "chilled" for transfer. Valium does not directly effect the smooth muscle of the cervix, but it helps to avoid patients "tensing up" and feeling uncomfortable during the transfer. General anesthesia (i.e. conscious sedation - same as for the egg retrieval) can also be used and may be recommended if it is to be a Day 5 transfer. After 5 days of progesterone, the cervical canal is a pretty tight squeeze.
