When we did our transfer I did request a copy of the lab results regarding the grade of the embryos. That is all we received.
Is there a form the lab uses after the eggs have been retrieved and before the embryos are made to document the characteristics of the egg. What type of information can we expect to see on this form?
Are there any other forms the lab documents on the embryos such as cleavage time of the embryos.

All U.S. IVF laboratories document the IVF process from retrieval of eggs to the final disposition of the embryos. The degree of detailed observations made on the eggs and embryos will vary from lab to lab. At a minimum, laboratories will document egg maturity, fertilization status, embryonic cell number or stage (i.e. blastocyst) and, for cell-stage embryos, the degree fragmentation/quality (i.e. grade). The convention is to observe the embryos once each day and document the above. Some laboratories record much more information about the eggs and embryos than the minimum described above, but this is usually represented by acronyms and other cryptic notations that only another embryologist would understand.
