Hi Dr. Jacobs,

Could you please comment on what the outlook for ovarian response would be in a healthy 36 yr old woman with an FSH of 6.94 and estradiol of 125 at cd3? I should add that I also had a sonohystogram on cycle day 8 of that same bloodwork cycle and was told I was about to ovulate. Could being earlier in the follicular phase than we thought cause the elevated E2 and not necessarily a poor prognosis for ovarian response?

36 years old. Married for 17 years. 1 son, 7 years old, conceived w/ ICSI. 2nd IVF w/ ICSI in June=No Fertilization. August cycle cancelled for poor response. Considering options.

An FSH of 7 Iu/L is fine, but on cycle day 3, an estradiol of 125 pg/ml is high.  It may falsely suppress FSH.  Probably a more accurate estimate of ovarian reserve can be made by counting the number of follicles visible on ultrasound on day 3.

Just a quick follow-up. Do you think that being further along in the follicular phase when the blood was drawn could cause the E2 to be high?

Yes, your estradiol level is lowest on cycle day 3, and rises to a peak a day before ovulation.
