Dr, I am 41yrs and have 'unexplained infertility'. Had a miscarriage in 2001, 1 try at IVF in 2004 (unsuccessful). Have (on my request, as my Dr says we have just as good a chance of success without it) had 2 iui's and about to have my third (and probably last). If this one is unsuccessful, is it worth me asking Dr for another try using some sort of medication? He has been reluctant to give any medication before, as he says I am ovulating and dont need it. Surely, due to my age, it is worth a try? He seems very worried about multiple pregnancy. Would appreciate your views.

Over the age of 40, the potential for a successful pregnancy is extremely poor, unless you use an egg donor.  Looking at the statistics, when we put embryos in the uterus, using eggs from women 40 to 44 years old, the implantation rate is 6.1%.  Women in the same age range who do become pregnant have an almost 35% risk of miscarriage.  Doing IUI is a very poor odds process for you, even worse without using medications to try to stimulate your ovaries.  Even with medication, I would not expect a good ovarian response.

I am 44 years old and had two good cycles on 600 amps of Follistim.  My ovaries produced 3 follicles on each side.  The right side doesn't count because the  tube was removed. Doing IUI has not been successful for me-but I had to try to beat the odds. If IUI doesn't work after this month, I'm going to switch to IVF and possibly use my eggs.  The odds are poor-but I like to be hopeful, even though it's not cost effective.
