What are the reasons for failed donor egg cycle

I just completed my first donor egg cycle and the 2ww, which ended in a negative result.
A little background about myself. I am 40 (41 next month), DH 47. We currently live in Malaysia and went through the procedure here. Wish were back home in Virginia though. I have had 2 failed pregnancies (July 08, Nov 08) and one failed IUI (Nov 09) all ended in "chromosomal abnormalities" trisomy 16 at 8-9 weeks. We skipped IVF and went straight to donor since age and high FSH (23) is a factor.
But this has failed too. My uterine lining was around 5.7 - 6mm pre-transfer and womb length 6.75 (dr. said its too small) but I am also petite. Donor is 25, produced 48 eggs of which 27 were fertilized and 15 embryos remain to be transfered. Dr. used two embryo's with a 3 day transfer.
Is the lining the issue? How can I improve it? Have been on 2mg progynova 3xday and cyclogest 2xday.
