Dear Doctor,
Im starting an IVF cycle on CD 21 w/ Lupron.
Is there any difference between starting Lupron on day 21 vs. day 2? Under what circumstances would you reccommend starting on day 21 vs. day 2?

For my patients I expect a normal ovarian response to stimulation, I start Lupron on day 21. For those patients who will be poor responders, I use a flare protocol, starting Lupron on day 2. Before Lupron shuts down secretion of FSH and LH, it causes the pituitary gland to dump all it has. This "kick starts" the stimulation
