lap then IVF or straight to IVF??

i was diagnosed with endo 10 yrs ago. i had a lap 7 years ago & was then on lupron for 2 months. i then got pregnant & miscarried after 2 weeks.

now i am 29 yrs old & my husband and i have been trying to conceive. after 4 unsuccessful iui's- the doc wants to do ivf.

i've already started on birth control & i'm waiting to do a saline sono on friday.

do you think a lap prior to ivf will help my chances?

I cannot advise you about laparoscopy without evaluating you and the studies already performed. Usually my decision about recomending laparoscopy is related to whether or not my patient produces an adhesion molecule in the endometrium felt to be important for embryo attachment.

Thank you doctor. How do you test to see if your patient produces an adhesion molecule in the endometrium? Is there sometime of biopsy?

There is only 1 commercial lab in the country which assays for the alpha-5-beta-3 integrin. They have patent rights on the reagents. I perform an endometrial biopsy 7 days after ovulation and send them the tissue.
