Before the local labs started cutting corners on the insulin assays, and the reports became unreliable, I would measure insulin levels on each blood sample of a 3 hour glucose tolerance test. Today, I make a clinical judgement from data derived from the medical history and physical exam.

thank you Dr. jacobs, for your reply. My current RE says that PCOS is determined by the cholesterol readings to glucose levels, that ratio determines insulin resistance.
I had my endrocronologist run the gtt, and she did a 2 hour test, my levels were elevated but the range might be at the border, right at the cut off. I am planning on returning to my endrocronologist.
I live in Richmond, VA and we have a DR. Nestler who has done extensive research on PCOS.
What types of tests do you run to determine if a patient has PCOS ?

Probably the most reliable test is the ultrasound appearance of the ovaries - theso calle "string of pearls" appearance of small follicles at the periphery of the ovaries. Good luck.
