follicle sizes out of sync

Hello.
Could you offer some advice on how you would manage an IVF cycle where the follicle sizes are all over the map?
My wife's latest scan, after 9 days on follistim, showed a lead follicle at 18, one at 14, two at 12, and a lot of smaller ones. E2 level was 1,885, which the nurse said was high. RE wants to stim at least one more day, but won't speculate on the ultimate number of mature eggs we might get.
Do you have no choice but to trigger when the lead is ready, or is there some way to buy time and let the others catch up? We are doing IVF primarily for PGD and are afraid we won't have enough to work with.

There are a number of stimulation protocols for IVF, and different criteria for the timing of hCG administration. If timing is right, even the small follicles can produce mature eggs. Good luck.
