IUI and male infertility

Hi Dr. Smith, My husband had a varicocele ligation in June of last year for a motility of 10% and a sperm count of of 17 mill. Four months post-op he repeated a semen analysis and results were great. Motility up to 53% and SC 15 mill. I am taking Follistim for chronic anovulation. We did our first IUI last month and the pre and post sperm washing SA results were very disappointing. Motility was back at 10% and SC ~20 mill. The IUI was unsuccessful. My Dr. said we would try IUI one more time and if his SA results were the same as last we should move on to IVF. Does this sound pretty reasonable? What are the chances of success doing IUI with a motility rate of only 10%? Thank you. Just seeking another opinion.

I agree completely with your doctor.

Varicocele repair only works in about half the cases and it can take up to 18 months to see improved SA results. It is remarkable that the SA results improved so much in only 4 months. However, if the 4 month follow-up SA was performed at the urologist's office, the dramatic improvement is less suprising. 'nuf said.

The chances of a successful IUI with 10% motility is virtually zero. Sorry, I'm sure that's not what you wanted to hear, but I think you have to be realistic.
