FRAGMENTED EGGS

My husband and I have been married twenty two years now and have been trying for a baby all of that time. I have had three ectopic pregnancies. I have tried IVF twice. Both time I have had a large number of eggs taken from the ovaries. Quite alot fertilise. Then quite a number start to fragment. My husband has anti sperm antibodies. So the last time we went for IVF, the clinicians had to perform ICSI. Two eggs fertilised and were good enough to be put back into the womb. But alas no pregnancy. My husband is 45yrs of age and I am 43 years of age.

Could the problem with the fragmenting of the eggs be due to the fertility drugs. Should I try again for IVF with a different fertility drug. I have had gonal f and metrodin. Are there any better drugs that I can try with IVF.

All stimulation medications are basically the same. They all contain the hormone FSH. Since the FSH in one medication is the same as in another, it doesn't make any difference what brand of medication you use. However, the medication must be used properly. The stimulation has to be monitored and the dose adjusted accordingly. This is not usually a problem in the hands of an experienced reproductive endocrinologist, as they are trained to use these medications.

When you first began to try to conceive, the antisperm antibodies may have been a problem. If all the sperm stick to each other, there isn't any left over to go and look for an egg. IVF/ICSI can overcome the antisperm antibodies problem, but excessive embryo fragmentation is a result of poor egg quality. I suspect that your fragmented embryos are a result of your age. Egg quality is dramatically affected by age. After 35, egg quality goes down hill quickly. Unfortunately, excessive embryo fragmentation is very common in women over 40. Because the live birth rate is so low (<5%), IVF is not recommended in women over 42. I would not recommend another cycle. Sorry to dash your hopes that a simple change of medication will help, but I don't think that's the problem.
