Clomid vs. Gonal F

My husband and I are back trying for our second child. I have PCOS and we conveived our son almost 4 years ago with Gonal F, HGC trigger, and and IUI. We moved and have a new RE and he gave us the choice to try Clomid or Goanl F with an IUI. We tried Clomid this summer and had 10 follicles and great numbers for my husband. Before we make the decision of either medication we have one question. Does either medication affect the quality of the follicules that are produced? Our gut is telling us to stick with the Gonal F because it worked before, but the concern of hyperstimulation is a slight issue.

Hello Deanna,
Gonal F is the better choice. It will not compromise the endometrium (which Clomid may do) but it does leave you at risk for multiples. The potential hyperstimulation may be quelled with IV fluids if needed and with oral fluids (8 - 10 glasses/day) of H20 beginning after the IUI. As you do have PCO it is OK to use Clomid even though your chances are improved with the GF. However I have seen multiples even with Clomid - so it may not really be that much "tamer". I understand that you have already tried 1X with the Clomid. A total of 3 cycles is OK to do. (I wouldn't do more.) In my practice I suggest IVF/blastocyst transfer for patients with PCO. These procedures allow us to evaluate the embryos, selectively transfer 1 or 2 at most and avoid multiples beyond twins. Remember - you have a child - your uterus has been "primed"- it would not be impossible for you to have multiples. Be aware of this.
