assisted hatching

Dr. Smith,

I am 32 years old and am beginning my second fresh IVF cycle. I have had one failed IVF day three transfer and one failed FET (frozen on day five).

I met with my RE yesterday to discuss our next try at IVF. He recommended some chanegs in my medication stimulation to get better quality eggs. I also asked if he would do assisted hatching on the embryos we transfer during the next cycle. He stated that he had not seen a statistically significant increase in pregancy rates with AH, and that it was a big "fad" a few years ago. He only does AH in about 5% of his current patients. However, he said he would do it if we insisted because it "couldn't hurt."

What are your thoughts on these comments. Are there any risks that I should be considering or worry as far as injury to the embryo. I feel that if it helps me achieve pregancy by even 1% it is worth it to me.

Your doctor is right that AH of Day 3 embryos has not been shown to make a difference, except in 3 groups of patients: (1) advanced maternal age; ie >38 years old , (2) one or more failed IVF cycles and (3) in FET cycles. Since you have had a failed cycle, I agree with your doctor, it can't hurt and it may help.

If performed properly, there are no risks to the embryo.

We routinely perform AH on blastocyst stage embryos, but thats for different reasons.
