The name for this is "polyspermy". To prevent more than one sperm from entering the egg, the protein coat (zona pellucida) that surounds the egg hardens up in response to the first sperm penetration. This is called the block to polyspermy. In your case, the block to polyspermy is not occuring.
I have seen this in a few patients over the years. After all or the majority of eggs are polyspermic on the first cycle, intracytoplasmic sperm injection (ICSI) is performed to prevent polyspermy in any subsequent cycle. I am very surprised that ICSI was not performed for your second cycle.
