Hi Doctor

I am very confused over my results for my donor egg cycle. My donor had 15 eggs retrieved - out of which today I find out only 5 were mature. Out of the 5 mature, all 5 fertilized. What could possibly cause so many to be immature? Clinic is trying to make me feel better that of the 5 mature, 100% of them fertilized. But I am wondering can these 5 embryos be strong with so many immature? Donor is 22 years old, started stims on Sept 1, retrieval was yesterday, 9/11. She was on Gonal F, Cetrotide, low dose LH. I am just feeling my cycle is doomed. ???

The donor probably took the hCG late. If the eggs do not have at least 36 hours to undergo a final maturation process, most of them will be immature a the time of retieval. We perform an hCG test on the day following the injection to see if the level is adequate and to determine if the donor took the hCG at the right time. If the donor's hCG level is low, she is grilled until she confesses (torture is not usually necessary). The retrieval is postponed until the 36 hours has elapsed. Can make for some late nights in the lab, but it works out well in the end.

Another explanation is that the hCG was administered when the majority of the follicles were small (i.e. too soon). The eggs inside the smaller folicles were unable to respond to the hCG and did not undergo the final maturation process. Those eggs retieved from the larger folicles may be O.K.

All 5 of the mature eggs fertilized! Woohoo! You guessed it. Damage control.

Your cycle is not doomed - yet. Hang in there. Give the 5 embryos a chance to develop. They may still be O.K.
